1. Phone consultation
First, we’ll start with an initial phone consultation to discuss the client’s caregiving needs. This will help everyone involved come up with the best plan to ensure the client’s needs are met and the agency can assign caregivers to the residence.
2. In-home assessment
Next, we’ll conduct an in-home assessment to fully understand the home and answer any questions the client may have. This assessment with an RN can also be done via Zoom, if requested.
3. Recommendation is given
Next, the nurse gives a recommendation on how to proceed and builds a personalized plan of care.
4. This plan is then initiated, followed by bi-monthly nurse evaluations
