Our Team
Advanced Care’s team of more than 250 professionals includes caregivers, care coordinators, registered nurses and administrative staff. Part of their work is helping families with Medicaid and insurance processes, which can be confusing to sort out alone.
Our team includes bilingual members, so Spanish-speaking families can talk with us comfortably.
In-Home Care Services
Every care plan is customized and changed as a client’s needs change. Services include:
- Personal care: hygiene assistance and help with dressing and undressing
- Home assistance: meal preparation, feeding assistance and light housekeeping
- Medication reminders
- Transfer and mobility: transfer assistance, fall prevention, help with walking and range of motion exercises
- Transportation: social outings, shopping, medical appointments and church
- Everyday tasks: getting the mail, appointment reminders, supervising home deliveries and personal business tasks

How Care Starts
Getting started takes four steps:
- An initial phone consultation to talk about your loved one’s care needs.
- An in-home assessment by a registered nurse. This can also be done over Zoom if you ask.
- The nurse makes a recommendation and builds a personalized plan of care.
- Care begins, followed by bi-monthly nurse evaluations.
Consultations and assessments before care begins are free. Our nurses also do an annual comprehensive assessment for each client. The details are on how it works.
