What Memory Care at Home Looks Like
Memory care at Advanced Care is everyday, practical support built around the person. Depending on the care plan, a caregiver can help with:
- Hygiene assistance and dressing
- Meal preparation and feeding assistance
- Medication reminders and appointment reminders
- Fall prevention and help with walking and transfers
- Conversation and company
- Getting the mail and supervising home deliveries
- Rides to medical appointments, shopping, church and social outings

A Plan Built by a Nurse
Every client starts with an in-home assessment by a registered nurse, who builds a personalized plan of care. After care begins, our nurses do bi-monthly wellness assessments and an annual comprehensive assessment. Care plans are adapted as a client’s needs change, which matters when memory loss changes over time.
Our caregivers are asked to report any noteworthy change in a client’s condition, so the family and the care team hear about it.
What We Do Not Do
Advanced Care is a non-medical in-home care agency. We do not diagnose or treat memory loss or dementia, and we do not provide wound care, medical equipment, physical therapy or occupational therapy. Questions about a diagnosis or treatment belong with your loved one’s doctor. The Alzheimer's Association also has information and support for families.
Support for Families
Our caregivers give your loved one personal interaction and help when you cannot be there yourself. Our team includes bilingual members for Spanish-speaking families.

Starting and Paying for Care
Starting care follows four steps:
- An initial phone consultation to talk about your loved one’s care needs.
- An in-home assessment by a registered nurse. This can also be done over Zoom if you ask.
- The nurse makes a recommendation and builds a personalized plan of care.
- Care begins, followed by bi-monthly nurse evaluations.
Consultations and assessments before care begins are free. Advanced Care accepts Medicaid, most HMOs, managed care organizations and other government programs, long-term care insurance, and private pay. See ways to pay.
