First, we start with a phone consultation. This will allow us to discuss and understand the client’s needs and help us ensure those needs are met. We can then assign caregivers to the residence to best accommodate the frequency and ensure the client has the support they need.
Then, we’ll begin an in-home assessment to make sure we understand the layout of the home and are able to answer any questions the client may have for us. We’re able to accommodate clients all across South Milwaukee, and a good portion Wisconsin. It’s important to note we are also able to complete this in-home assessment for senior care with an RN over Zoom if this is more convenient for those involved.
Finally, we’re able to give a recommendation on the best course of action for the client’s home care needs. This way, we can build a personalized plan of care based on the information gathered.
From here, the plan is initiated, including bi-monthly nurse evaluations to make sure the senior care plan is working well for all involved.
